Terms and Conditions of Stress Free woman
By using this site, you ("the Client") have agreed to the Terms and Conditions ("Conditions") of use with Stress Free Woman who provide their service ("the Service") on this website (StressFreeWoman.com). The Service is delivered by to the Client by the physiotherapist (the Physiotherapist) working in conjunction with Stress Free Woman. Please do not use this site if you do not agree to all of the Terms and Conditions of this agreement. We may revise the Terms and Conditions in the future.
By nature of the fact that the Physiotherapist is not in direct contact with the Client, the Service cannot meet the needs of every Client. Stress Free Woman will endeavour to provide online treatment based on the information supplied in the Symptom Summary and Bladder Dairy online forms, along with answers to any other questions asked by the Physiotherapist during the online consultation.
It is the Clients responsibility to give accurate and honest answers to questions on the forms and those asked by the Physiotherapist. However if the Physiotherapist feels they cannot provide a reasonable level of service, perhaps because the Clients' condition is inappropriate to be treated online, the Physiotherapist will not proceed with the treatment and suggest that the Client take alternative courses of action.
Online physiotherapy treatment can never be as comprehensive as a face-to-face appointment with a Chartered Physiotherapist.
Provision of online treatment
The Service will only provide online treatment if the Client is 18 years or over. Stress Free Woman’s website gives details about types of symptoms and underlying conditions that the Client may be experiencing that would be unsuitable for the online consultation provided by the Service. The website advises these Clients to consult their doctor.
The Symptom Summary form that all prospective Clients complete also contains questions that screen out any Clients for which the Service would not be suitable.
Clients who are suitable for the Service are directed to payment methods and given access to complete the Symptom Summary and Bladder Dairy. Once the Client has completed and returned these forms the Physiotherapist will view the information and contact the Client to arrange an online consultation.
We operate a cancellation policy whereby if the Client cancels less than 24hours before their online consultation no refund is given.
Refunds Initiated by Stress Free Woman
If on viewing all online forms and after receiving payment the Service decides we are unable to provide a reasonable treatment online, we will give the Client a full explanation and refund.
We will endeavour to deliver, by an online video, audio (or video and audio combined) a consultation that will include lifestyle advice and personal treatment programmes for the Client. This will be arranged at a time mutually agreed between the Client and the Physiotherapist. The Physiotherapist will have viewed the Symptom Summary and Bladder Diary prior to the online consultation.
We use Skype for our consultation, which is available for Windows, Mac, and most mobile devices. You will need to download this prior to our call. Should our session be disrupted or ended by connectivity problems, caused by lack of signal strength or for whatever reason, then the session could be re-scheduled for a different mutually convenient time and date at no expense to you
The Service will summarize the online consultation and forward, to the Clients email address, the content of the consultation with links to the appropriate Stress Free Woman’s online video tutorials in line with the Clients clinical need after the Physiotherapist’s assessment of their symptoms.
The Client will be contacted 6 weeks after the initial online consultation via email and requested to complete the ‘Symptom Update’ form. The Physiotherapist will review this document and send via email to the Client suggestions for ways of progressing exercise programmes and advice about further lifestyle changes that may enhance the Clients rehabilitation.
Arrangements can be made with the Physiotherapist should the Client request further support via follow up online consultations. ‘Master Class’ consultations are also available with video tutorials which may include for example guidance around the use of a muscle stimulation unit or pelvic floor rehabilitation for a sportswoman.
Links given to the Client to video tutorials will be password protected ensuring any tutorial the Client has access to is in line with their clinical need.
Dealings with outside agencies
Neither the Service nor the Physiotherapist benefits from any financial reward from any company should the Client purchase any products suggested by the Physiotherapist during the online consultation. The Service will only advise the Client about products that may be helpful for their presenting condition. Products discussed will be in line with those that are commonly used by women’s health physiotherapists in the UK. Some of these products may be licensed for use outside the UK. Whilst every effort will be made the Service cannot guarantee that use of any particular product will result in an improvement in the Client’s symptoms.
Clients must take the responsibility to read manufacturers guidelines around the safe use of any product they purchase after discussion with the Physiotherapist.
Links to / from other sites
The Service may provide, links to other World Wide Web sites or resources. Because the Service has no control over such sites and resources, the Client acknowledges and agrees that Stress Free Woman is not responsible for the availability of such external sites or resources, and does not endorse and is not responsible or liable for any content, advertising, products, or other materials on or available from such sites or resources.
Legal liability disclaimer
To the fullest extent permitted by applicable laws, and except in respect of death or personal injury arising from The Service’s negligence, Stress Free Woman hereby excludes liability for any claims, loss, demands or damages of any kind whatsoever with respect to the Service, and any information, content, advertisements or products provided or distributed through the Service including, without limitation, direct, indirect, incidental or consequential loss or damages, whether arising from loss of profits, loss of revenue, loss of data, loss of use or otherwise and whether or not the possibility of such loss has been notified to Stress Free Woman. The foregoing will apply whether such claims, loss or damages arise in tort, contract, negligence, under statute or otherwise. If you are a consumer your statutory rights, if any, are not affected.
The Service being provided is directed and appropriate only to the Client who has made contact with Stress Free Woman. It is not designed, permitted or appropriate for the Service to be used by any unidentified person or persons. Stress Free Woman will not have any contractual other legal obligations to or responsibilities for any other unidentified User. By using the Service you agree to the above, and in the event of a claim, action or demand being made by an unidentified User against Stress Free Woman you agree to indemnify Stress Free Woman against any such claims, actions, demands or liabilities, including any legal fees incurred by Stress Free Woman in dealing with any such claim, action or demand.
These Conditions constitute the entire agreement between you and Stress Free Woman and govern your use of the online service, superseding any prior agreements between you and Stress Free Woman. The conditions and the relationship between you and Stress Free Woman shall be governed by the laws of England.
You and Stress Free Woman agree to submit to the exclusive jurisdiction of the English courts. Any failure by Stress Free Woman to exercise or enforce any right or provision of the Conditions shall not constitute a waiver of such right or provision. If any provision of the Conditions is found by a court of competent jurisdiction to be invalid, the parties nevertheless agree that the court should endeavor to give effect to the parties' intentions as reflected in the provision, and the other provisions of the Conditions remain in full force and effect.
The Service is committed to protecting your privacy. To best achieve this, we provide this notice explaining our policy within this area.
Collection of data
The Service will require an e-mail address in order to contact you. We are aware that some email addresses contain your name which is classified as personally identifiable information
The Service stores your Symptom Profile, Bladder Diary (online forms) and any email correspondence between yourself and the Physiotherapist linked only to your email address. We do not store any other personally identifiable information. To keep an accurate record of advice given and discussions that took place between the Client and the Physiotherapist, an audio recording of consultations and correspondence is stored on a FileVault encrypted drive, as well as on Backblaze for backup purposes.
In order to offer the most appropriate clinical advice the Physiotherapist will have access to view the online forms and email correspondence. The Service may commission an administrator to assist in data collection for audit purposes.
Disclosure of data
The Service may use the information received in the forms and view the outcomes of the online service Clients receive to provide audit information. The Service will study statistical results and may produce summary reports e.g. to review the average age of our Clients, to view Client satisfaction with the Service provided and to measure changes in a Client’s quality of life. We will also use the feedback from the Client about changes in their symptoms to evidence the effectiveness of the Service and make changes to enhance future Clients experience.
The Service will not disclose the email address you or any of the data collected in the online forms without your permission except under special circumstances, for instance if the Service is legally obliged to provide any such information or disclosure by police request or court order.
Correcting / Updating / Storing Information
The Client will be given a copy via email of their Symptom Summary form and Bladder Dairy and all email correspondence forms between the Service and Client. The Service will keep the aforementioned forms and emails for the duration of contact with the Client and for a period of time of 7 years after the Clients discharge.
Stress Free Woman Security Policy
Whenever the Service handles your email address and online forms, regardless of where this occurs, the Service takes every precaution to ensure that your information is treated securely,
No data transmission over the Internet via email can be guaranteed to be 100 per cent secure. As a result, while we strive to protect your information, the Service cannot ensure or warrant the security of any information you transmit to the Service and you do so at your own risk.
Physiotherapists working in conduction with the Service operate under the strict standards of confidentiality as laid out by the professional standards of the Chartered Society of Physiotherapy and the Health Professions council.
As such, no personal Client details will ever be disclosed by the Service or by a physiotherapist working with the Service to any other party.
